COMMUNITY RELIEF

Each grant applicant will be reviewed by The PATH Fund's Special Grant Committee. 

Grants will be awarded by The PATH Fund on an objective and nondiscriminatory basis upon reviewing the application and ascertaining any additional facts and circumstances as necessary. 

1. Grants will be made electronically or by paper check in the form of lump sum distributions to the recipient, or as determined by The PATH Fund based on the needs of the particular recipient. 
2. Grant recipients may be required to provide The PATH Fund with additional documentation that the grant was used in a manner consistent with the terms of the grant. All data is kept confidential and secure. 
3. Grants issued based on the order the applications were received and based on applicants need up to the amount being given over said period of time. 
4. Grants will be awarded once per month dependent on number of applicants and funds raised. 
5. No grants will be made to individuals making over $150,000 in 2019. 
6. No grants will be made to individuals who are currently employed full time.
    

In order to qualify for a grant from the PATH Fund’s COMMUNITY RELIEF Grant Program, applicants must: 

• Submit PATH Community Relief Fund Application completed in full.
• Provide financial information to receive funds if awarded grant.
• Provide completed W9 Tax Form.
• Agree to post social media platforms to help PATH raise further funding.
• Proof of Participation: Copy of program, show photo, website listing. current resume and/or Proof of Union Membership.
